Chapter 21 – Diplopia. Episode overview: 1) List the differential diagnosis (critical emergent, urgent) for Diplopia. ▫ Including at least 7 causes of binocular.

Generally, macular pathology can be noted on biomicroscopic examination and can cause distortion of the Amsler grid or abnormal optical coherence tomography OCT findings. Some adults are also able to suppress their diplopia, but their suppression is rarely as deep or ibnocular effective and takes much longer to establish, and thus they are not at risk of permanently compromising their vision.
Skew Deviation Patients with skew deviation complain of vertical and occasionally torsional binocular diplopia. Objective To study the causes, diagnosis and treatment in a case series of binocular diplopia. Neuroimaging is often unrevealing but can be initiated if the palsy does not remit. From This Paper Figures, tables, and topics from this paper. Continuing navigation will be considered as acceptance of this use.
Base-out prisms eliminate diplopia effectively but may cause diplopia at near unless they are applied on the distance portion of the glasses only.

Patients with long-standing, stable strabismus for more than 6 to 12 months could consider strabismus surgery. Patients with a fairly comitant deviation may prefer prismatic correction. That involves viplopia one eye to see which symptoms are evident in each eye alone.
Evaluation and Management
Strabismus induced by radial keratotomy. Careful observation of the pupil will show constriction with attempted abduction. The advantages of Scotch Satin tape are the low cost, reasonable appearance, and ability to patch only portions of the lens where diplopia occurs. Efforts must first be made to identify and treat the underlying cause of the problem. Exophthalmos Enophthalmos Orbital cellulitis Orbital lymphoma Periorbital cellulitis.
This item has received. The most frequent diagnosis was sixth nerve palsy The appropriate treatment for dde diplopia will depend upon the cause of the condition producing the symptoms. Orthoptic training using base-out prisms for near viewing, convergence exercises using stereograms, home computer orthoptics, and changing fixation from distance to near targets may improve symptoms.
Patients note vertical, oblique, or torsional diplopia worse in downgaze, and they often adopt a contralateral head tilt to counteract the diplopia.
Binocular diplopia in a tertiary hospital: Symptoms, often worse in the morning, may start with complaints of irritation and blurry vision followed by slowly progressive binocular diplopia.
These patients typically binoculwr not benefit siplopia prism therapy, vitreoretinal surgery, or strabismus surgery. A skew deviation or upbeat nystagmus in upgaze may be present, as the MLF also carries vertical eye movement information and input from the cerebellum to the midbrain. Demyelination often causes INO in younger adults and microvascular infarction typically causes INO in elderly patients. This complication was much more common with the early Baerveldt implants than with the Ahmed or Molteno implants.
Visual disturbances and blindness Vision. While this ability to suppress might seem an entirely positive adaptation to strabismus, in the developing child this can prevent the proper development of vision in the affected eye resulting in amblyopia.
Treatment options include eye exercises, wearing an eye patch on alternative eyes, prism correction and in more extreme situations, surgery or botulinum toxin. Only one third of patients had a spontaneous resolution, and half of them did not have a good outcome despite of treatment.

Globe Fibrous tunic Sclera Scleritis Episcleritis. Patients complain of horizontal diplopia worse in the distance, and examination reveals xausas esotropia that increases with ipsilateral horizontal gaze.
Showing of 12 references. This is usually vertical diplopia due to small fusional amplitudes and may be correctable with a slab- off prism from the more-minus or less-plus lens.
KellyD H Haight Journal of cataract and refractive surgery The mean onset of diplopia was Conclusions The most frequent causes of binocular diplopia were cranial nerve palsies, especially the sixth cranial nerve, followed by decompensated strabismus.
Cataract Congenital cataract Childhood cataract Aphakia Ectopia lentis. This patient can abduct cauaas right eye normally. Refractive surgery and diplopia. Postoperative residual hyperopia may ginocular breakdown of a previously controlled accommodative esotropia.
In patients with spasm of the near reflex, there is variable esotropia and abduction.
A patient blnocular an isolated, dilated and nonreactive pupil with normal extraocular movements and eyelid position does not have a third nerve palsy. The ability to suppress is to be found particularly in childhood when the brain is still developing. The deviation either resolves within the next several days to weeks or evolves into a permanent restrictive strabismus.
